Scheduling for New Patients
If you’re a new patient with GFMA, you will need to contact us to book a new appointment. Once you have booked your appointment, you will receive the new patient questionnaire electronically via email. Please make sure that you complete the questionnaire in its entirety.
For all existing patients, see below.
Scheduling for Existing Patients
If you’re an existing patient with GFMA, feel free to request an appointment below with the provider you saw at your last appointment. Appointments appear below based on availability. Select the date and time that you wish to schedule the appointment. Enter your birthdate, gender, if you have seen the provider before, and include the specific reason for the visit (e.g., annual wellness visit, fever, sore throat, diabetes, high blood pressure, etc.). Click the arrow and then enter in your First & Last name, email address, and phone number. Click “Finish” once you are done. You will receive an email with your appointment confirmation and any paperwork needing to be completed in the confirmation email. Please make sure that you complete the paperwork in its entirety.
