Wheat Ridge Geriatric Care Services

An Extra Level of Care for Seniors

When your children were little, you probably took them to a pediatrician. After all, babies and children have specialized needs that differ from those of adults. On the other end of the spectrum, older adults also have different medical concerns than those in their 20s or 30s. Many people have yet to learn that there are providers who have an extra level of care for seniors. At Geriatric & Family Medicine, our primary care providers are geriatric specialists. We specialize in the healthcare needs of people who are aging, especially those 65 years old and up.

A geriatrician can be helpful if you:

  • Suffer from multiple medical conditions
  • Find that treatment for one medical condition negatively affects a second condition
  • Are experiencing functional decline or physical frailty
  • Have diseases associated with aging, such as dementia, incontinence, or osteoporosis
  • Manage multiple medications, especially if they are causing side effects that affect your well-being

We specifically concentrate on the following areas that frequently affect older adults:

  • Advanced Directives (i.e., Colorado MOST form)
  • Arthritis
  • Alzheimer’s
  • Balance Issues
  • Cancer
  • Dementia
  • Diabetes
  • Depression
  • Falls
  • Frailty
  • Hearing loss
  • Heart disease
  • High blood pressure
  • Incontinence
  • Insomnia
  • Osteoporosis
  • Vision loss
  • Walkers/wheelchairs

Chronic Care Management

We pride ourselves in ensuring that we are on the same page regarding your health goals, keeping in mind that these goals often change over time. We pay more attention to your overall quality of life. At Geriatric and Family Medicine, we act as a point of contact for your healthcare needs. We keep track of complex drug interactions and prioritize treatments for people dealing with several conditions while fostering healthy aging. Aging presents unique physical, cognitive, emotional, and social challenges. We offer chronic care management, a service covered by Medicare for patients with two or more chronic conditions. A team member from our office will check in with you once a month for 20-40 minutes to ensure we are meeting all of your needs. In the event that a patient needs to deal with palliative care or end-of-life care, we can help facilitate those conversations and referrals.
