Chronic Care Management
We pride ourselves in ensuring that we are on the same page regarding your health goals, keeping in mind that these goals often change over time. We pay more attention to your overall quality of life. At Geriatric and Family Medicine, we act as a point of contact for your healthcare needs. We keep track of complex drug interactions and prioritize treatments for people dealing with several conditions while fostering healthy aging. Aging presents unique physical, cognitive, emotional, and social challenges. We offer chronic care management, a service covered by Medicare for patients with two or more chronic conditions. A team member from our office will check in with you once a month for 20-40 minutes to ensure we are meeting all of your needs. In the event that a patient needs to deal with palliative care or end-of-life care, we can help facilitate those conversations and referrals.
